You are billed when an envelope is created, not when it is signed. If your signer declines, ignores it, or you void it, it still cost 25 cents. A request that fails validation is never charged, because no envelope was created.
Two signers on one document is still one envelope. Text and email to the same signer is still one envelope. The first 25 are free once, on the sandbox, not every month. Nothing here is stored or sent anywhere.

When the 25 free sandbox envelopes are used up. Until then there is nothing to enter. The card unlocks both more sandbox envelopes and live sending; there is no separate decision point. Live envelopes are 25 cents, and sandbox envelopes after the free 25 are 25 cents too, on their own line of the invoice.
Monthly, on the anniversary of the day you activated paid billing. Activate on the 14th and you are invoiced on the 14th of every month. If your day does not exist in a month, it bills on that month's last day. You keep a primary card and a backup card; if the primary fails, the backup is charged.
Every account has an approved monthly envelope limit, which you set from your own estimate when you activate paid usage and can raise yourself up to 10,000 a month. You can also set a lower cap of your own; that one is a hard stop, and you can lift it in one click. Above 10,000 a month, a person approves the increase.
The envelope still cost 25 cents, because it was created and delivered. That is why the sandbox exists: get the flow right there, then create live envelopes you actually intend to send. Use an idempotency key so a retried request cannot create and bill two envelopes.
No. Use four today and the other 21 are there in six months. They do not refill either; it is a one-time allowance per account.
There are no plans. One rate, per envelope, billed monthly on what you sent. If you need something a per-envelope rate cannot express, write through the contact form and say what it is.
